What is BHRT?

Bio-identical hormone replacement therapy (BHRT) is hormone replacement therapy using bio-identical hormone products which have the same molecular structure the hormones naturally produced by your body. BHRT products are made from plants (yams or soy) and are generally well tolerated. The Endocrine Society has defined bioidentical hormones as “compounds that have exactly the same chemical and molecular structure as hormones that are produced in the human body.” There are a number of commercially available forms of bio-identical hormones as well as custom compounded formulations.

Compounded BHRT

Compounded medications require a written prescription from a licensed medical provider. Prescriptions filled in a compounding pharmacy are prepared, mixed, and assembled according to the specifications of the prescriber based on the individual needs of the patient and can be more specifically dosed for each patient. Compounding plays an important role in providing drugs to meet the individualized needs of patients that cannot be met by a commercially available FDA-approved preparation. It may provide a different strength, dose, or delivery system, or it may allow a medication that is otherwise poorly tolerated to be essentially reformulated and delivered in an alternative form (eg, capsules, vaginal creams or troches, suppositories, nasal sprays, sublingual drops, topical gels, rapidly dissolving tablets, or transdermal gels). Compounded hormone preparations are not FDA approved are not required to undergo the rigorous safety and efficacy studies required of FDA-approved HT and can demonstrate wide variation in active and inactive ingredients.

Testosterone-Replacement Therapy (TRT)

Initial Visit: $250

Price includes: initial visit and exam, initial lab, referral to certified strength and conditioning specialist for personalized diet and strength training recommendation, in-office bioelectrical body scan for body fat/lean muscle evaluation, first month of medication and supplies, Vit D and B12 injection.

Your first visit will take place at the practice location and will be in person with our provider. You will discuss your symptoms and our provider with conduct a physical examination as well as discuss benefits and risks of TRT. If you decide to move forward with the treatment plan, lab will be drawn at your visit and results are generally returned within 48 hours. Our provider will call to discuss your lab results and, if applicable, your medications will be ordered from our accredited compounding pharmacy and shipped to directly to your home for self-administration. Unless otherwise indicated, follow up labs are required every 3 months ($75) until your levels have stabilized and then generally twice yearly.

Monthly Subscription: $160

Subscription price includes: injectable medications and supplies (shipped to your home), Monthly Vitamin D and B12 injection, follow up visits/telemed appointments, bio-scan monthly for tracking of muscle mass change/fat loss.

Are there risks?

A relatively small number of men experience immediate side effects of testosterone supplementation, such as acne, disturbed breathing while sleeping (worsening sleep apnea), breast swelling or tenderness, or swelling in the ankles. It may also cause an increase in red blood cell (RBC) counts, which could increase the risk of clotting; which is why labs are required on quarterly basis. If you experience an increase in your RBC/hematocrit, you will need to donate blood on a regular basis or your therapy may need to be changed to another dose/formulation.

There may also be risk for cardiovascular events and development or worsening of prostate cancer. However, for men who have significant deficiency, use of TRT has not shown any greater/increased risk of these events. Your required labs will help to determine if your TRT is effecting your prostate by measuring your PSA. Initial spike in your PSA can happen after starting TRT but, if it remains elevated, you will be referred to urology for further evaluation. Regardless, men and their healthcare provider should still weigh the risk vs benefit of starting/continuing long-term testosterone replacement therapy.
